Overview

Provides the supervision, coordination and guidance of the day-to-day Assisted Living/Memory Care (AL/MC) activities on a designated shift for all AL/MC and as may be required by the
Nurse Manager to ensure that the highest degree of quality care is maintained at all times.

Responsibilities

1. Provides guidance and leadership to CNAs/RAs by directing daily assignments,
answering questions, teaching techniques and providing more advanced analysis and
interpretation.
2. Oversees staff scheduled and acts appropriately to maintain adequate staffing levels.
3. Ensures that all personnel on designated shift comply with all policies and procedures
established by the village, and the current state and federal rules, regulations and
guidelines that govern licensed HFA homes.
4. Assist in the development, monitoring and updating of individualized care plans.
5. Willingly works the floor when the need arises, assisting with CNA/RA duties.
6. Maintains cleanliness, security and supply of medications, supply rooms and equipment
and documents accordingly.
7. Participates in the development, maintenance and implementation of VEH Performance
and Improvement Programs.
8. Helps with the development and implementation of state plans of correction.
9. Completes Incident/Accident reports as necessary.
10. Assists nurse managers with the Quality Improvement Conferences (QIC) and
performance evaluation process.
11. Cooperates and coordinates activity programs with the AL/MC routines.
12. Admits, transfers and discharges residents as required.
13. Participates in the hiring process by interviewing perspective staff for the
afternoon/midnights shifts.
14. Performs administrative duties such as completing medical forms, reports, studies and
charting as necessary.
15. Identifies and assesses unit problems, resolving and/or making recommendations for
resolving and reporting to the Nurse Manager.
16. Makes daily staff assignments for resident care, breaks, treatment and tasks after
considering resident needs and the individual talents of staff.
17. Conducts rounds frequently to maintain awareness of what is occurring on the units and
to ensure that the staff is properly administering the prescribed treatments and to
evaluate the residents’ physical and emotional status.

Qualifications

At least one-year supervisory/lead experience. Having a CNA certification preferable but not required

Ability to operate a computer
