The ideal leader of this assisted living community will have clinical experience and is responsible for the coordination, direction, training, and management of staff, ensuring the needs, safety and comfort of the residents are met.    Assesses and monitors resident’s condition, and assist in performing resident care services as needed. A Licensed Practical Nurse (LPN) or Registered Nurse (RN) is preferred, but not required.  Associates or Bachelors degree in Health Care Administration, Nursing, Gerontology, Social Work or the equivalent experience.

Responsibilities

Responsible for providing hands-on leadership in the day to day operations of Assisted Living/Memory Care at McFarlan Home. Coordinates, directs, leads and supervises the Assisted Living/Memory Care, and community staff in order to fully meet resident needs.  Oversight of resident care and services.  Communicating with family members and service providers.  Ensuring compliance with state licensing regulations for Home for Aged. Maintain an environment consistent with PVM mission, vision, values, for residents, staff, and partnerships. Responsible for developing/carrying out strategic initiatives and action plans that are in alignment with PVM’s system-wide Strategic Objectives and Goals. Acts as Site Coordinator for Risk Management.

 

ESSENTIAL FUNCTIONS

 

  1. Conduct and administer fiscal operations, including accounting, planning budgets, authorizing expenditures, establishing rates for services, and coordinating financial reporting.
  2. Conduct supervisory activities such as hiring and firing staff, providing employee orientation and training, creating work schedules, coaching or disciplinary action, and approving employee time sheets for department.
  3. Coordinate the resident functional assessment process including the development of and monitoring of Resident Care Plans.
  4. Review and analyze facility activities and data to aid planning and cash and risk management and to improve service utilization.
  5. Plan, implement and administer programs and services in department including personnel administration, training, and coordination of care and other department staff
  6. Develop and implement policies and procedures for the department.
  7. Develops the strategic direction and long-range initiatives for the department.
  8. Maintain awareness of advances in medicine, technical issues, government regulations, health insurance changes, and financing options for the department.
  9. Ensures compliance of all state, regulatory, administrative, labor and legal regulations, requirements, laws, etc. for the Village
  10. Analyze data and submits reports (verbal and written)to inform management of the status and implementation plans of programs, services, and quality initiatives.
  11. Maintain, updates, and ensures compliance with all legal, regulatory, administrative, labor and legal requirements, laws, and technical issues.
  12. Contribute to local community organizations and appropriate professional organizations (state and national). Provides advocacy on key public policy matters at the request of PVM government relations representatives.
  13. Administers operating policies, procedures and programs
  14. Act as Risk Management Coordinator in line with PVM’s Risk Management Working Plan, in conjunction with PVM’s Director of Risk Management.
  15. Participate, as needed, in resident/family conferences and addresses concerns.
  16. Proactively maintain a compliance ready environment. Develop and implement plan of correction based on survey citations.
  17. Review and approve documentation, charts and records for resident files
  18. Meets regularly with Resident Association or related group to solicit input and respond to concerns
  19. Responds to calls and emails in a timely manner
  20. Observes and monitors resident condition, including care, diet, psychosocial and spiritual needs by making rounds and make appropriate referral.
  21. Attend and participate in all appropriate meetings and training opportunities in order to keep informed on current information and skill development.
  22. Follow established policies and procedures including but not limited to:
    • Presbyterian Villages policies and procedures.
    • Safety policies and procedures.
    • Federal, state and local regulations.
  23. All other duties as assigned

Qualifications

Education:

  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) preferred
  • Associates or Bachelors degree in Health Care Administration, Nursing, Gerontology, Social Work or the equivalent experience.

 

Experience:

  • Five years’ experience working within the Assisted Living environment
  • Understanding of regulatory compliance with Home for the Aged licensing
  • Supervisory experience required

 

Computer Skills:

Ability to operate a computer for purposes of timekeeping, education, developing and implementing reports, and any additional databases for successful completion of all other duties.  Proficient in Microsoft Office.

 

Certificates & Licenses:  

Licensure as a Licensed Practical Nurse or Registered Nurse preferred

Valid driver’s license and reliable transportation

 

Other Requirements:

Ability to participate in manager on-call rotation

Ability to work varying hours including evenings and weekends as needed

Ability to respond to emergencies via phone or in-person, as necessary
