Overview

Coordinates, directs, leads and supervises the Independent Living (IL), Assisted Living (AL) and Memory Care (MC) Unit (s) in order to fully meet the resident's needs.

Responsibilities

    1. Assist in the assessment, development, monitoring and updating of individualized care plans.
    2. Monitor all aspects of resident condition, care, diet and physical activity by making rounds (with or without the physician). Facilitate transfers to hospitals or triage centers.
    3. Record patients’ medical information and vital signs.
    4. Monitor, record, and report symptoms or changes in residents’ conditions.
    5. Maintain accurate, detailed reports and records. Review, correct and authorize documentation, charts, records, etc.
    6. Consult and coordinate with healthcare team members to assess, plan, implement or evaluate resident care plans.
    7. Perform administrative or managerial functions, such as taking responsibility for unit’s staff, budget, planning, or long-range goals.
    8. Conduct supervisory activities such as hiring and firing staff, providing employee orientation and training, creating work scheduled, coaching, and disciplinary action.
    9. Participate in care conferences, consulting with the team caring for the resident, to evaluate the resident’s needs and plan for continuing services.
    10. Direct or coordinate infection control programs, advising or consulting with specified personnel about necessary precautions.
    11. Develop and implement plan of correction based on survey citations.
    12. Attend and participate in all appropriate meetings and training opportunities in order to keep informed on current information and skill development.
    13. Follow established policies and procedures including but not limited to:
        • Presbyterian Villages policies and procedures.
        • Safety policies and procedures.
        • Federal, state and local regulations.
        • Continual preparation for the annual state licensure survey and conduct monthly in-services so all associates are prepared and up to date.
        • Supervise and train all medtechs to ensure they are administering and documenting all medications as ordered.
        • To be on call for any resident, family or staff concerns as needed.
        • To lead yearly(or as needed) care conferences with families along with the interdisciplinary team.
        • All other duties as assigned

Qualifications

Education :

Associates Degree from an accredited school of nursing

Bachelor’s Degree in Nursing preferred

 

Experience :Six months to one year geriatric experienced preferred

 

Computer Skills: Ability to operate a computer.

 

Certificates & Licenses:

Must have ability to operate a computer

EMR experience preferred.

 

Other Requirements:

Ability to work holidays, evening and/or weekends

Current MI RN License
