Summary: The Service Coordinator assumes responsibility for coordinating programs and services to help residents maintain a good quality of life and age in place. Assisting in assessing and identifying resident and family needs and outside resources to meet those needs. Areas of assistance may include physical, mental, psychological and/or social as it relates to aging and moving in/moving out.
Perform any combination of duties to maintain resident’s health and well-being by assisting them with daily living activities such as grooming, eating, dressing, and bathing. Also maintain cleanliness of resident rooms/units and common areas including kitchen and laundry. Assist food service department including the preparation, service and clean-up of food service to residents, visitors, and staff.
Assists in the implementation of the Wellness department in the Assisted Living, Independent Living, Memory Care and Skilled Nursing environment. Responsible for group and one-on-one activities.
